Registered Nurse-Team Lead/Case Manager

The PRRC program is designed to support recovery and integration into meaningful self-determined community roles for Veterans challenged with serious mental illness and severe functional impairment. The program is curriculum based and specifically designed to teach the skills necessary to an active, independent and healthy lifestyle. Patients are likely to present with co-morbid medical conditions, functional decline and unstable living conditions requiring a case management approach in addition to education and training. The RN Team Lead/ Case Manager applies the nursing process and evidence-based practice in collaboration with the PRRC Treatment Team and Expanded Team (family/caregiver, internal and community-based services who provide care to the patient). PRRC provides patient and family health education and rehabilitation services with a focus on self-management, prevention, and wellness, based on the patient's goals and using a Recovery Model approach. The PRRC Team Lead RN is responsible for demonstrating and maintaining clinical expertise and knowledge of mental health evidenced based treatment, chronic disease management, preventive health, wellness and recovery principles to improve the quality of patient care. Job Functions:
1. Takes a leadership role in utilizing problem-solving methodologies to resolve system and/or process problems that impact on resident care or staff performance at the program or service level that is supported by documented evidence of:
a. Assuming responsibility for meeting an identified need; b. Developing and implementing the plan to meet the identified need; and c. Evaluating and providing evidence of improved resident care or staff performance relative to the identified need. 2. Demonstrates role modeling, leadership and advanced clinical expertise to improve the delivery of patient -centered care through the use of the nursing process for patients requiring specialized/complex care. 3. Develops creative, innovative strategies to resolve problems and/or promote positive outcomes in collaboration with nursing/medical center leaders. 4. Demonstrates sustained advanced ability to assess and document the needs of outpatient mental health patients in a self-directed manner. Reassesses patient changes and accordingly documents patient care. 5. Responds and supports emergency responses in MHS. Evaluates response and recommends strategy to improve outcomes. 6. Collaborates with clinical staff in accurately discriminating among multiple patient problems/system issues to focus on problem areas in treatment team meetings and meetings with administrative staff. 7. Acts as a consultant in clinical practice or in area(s) related to practice role responsibility and specifically education related to serious mental illness and the Recovery Model. 8. Provides patient education regarding medications, medication renewal process, mental health and medical disease processes, and documents patient level of understanding and outcomes. 9. Evaluates and updates education programs for PRRC Veterans providing psychosocial skill based and wellness enhancing training using individual and group formats. Leads the PRRC Team in developing creative and innovative programming for Veterans. 10. Provides age-specific care for adult, older adult (65-74) and geriatric (over 75) patients/groups including the following elements related to the aging process:
promotes independence involves family or designated caregiver with care for the veteran, provides assistance to patients to cope with loss and/ or compensate for changes affecting mobility, eyesight, hearing, and instructions for accessing emergency care. 11. Models person centered recovery technology as the philosophy of care and documents in Mental Health Suite in timely and effective manner. 12. Demonstrates knowledge and/or required safety skills in area of assignment, and, when appropriate, in the following:
emergency preparedness plan, hazardous materials, fire plan, equipment failure emergency plans and utility outage response procedures for the area. 13. Maintains all practice specific, specialized competencies and mandatory reviews. 14. Systematically evaluates current practice, formulates outcomes for groups of patients and/or organizational processes within area of expertise. Proposes revision based on evaluation of outcomes during team meetings and meetings with administrative staff. 15. As required, administers and documents medications and treatments according to policy. Work Schedule:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
-Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. -Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience. -Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. -Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. -Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent is expected to use proper preventive technics to perform duties. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
