Licensed Practical Nurse (Community Living Center)
The incumbent will be part of the Community Living Center healthcare team.
He/she will demonstrate the ability to respond to the Veteran's nursing needs for nursing, medication, and equipment-assisted care; and to recognize deviations from normal.
The incumbent will have the ability to conduct group activities for patients and families and the ability to work effectively with patients, family and other staff members and to respond appropriately to emergency situations.
He/she will incorporate acceptable, established customer service standards into practice; demonstrate knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures.
The incumbent will provide observation and documentation of the patient's response to medication administered and the reporting of any noted change in patient's condition to the RN.
Further, the incumbent will provide a full range of nursing care to patients with a variety of physical and/or behavioral problems, as well as, demonstrating accountability for meeting and maintaining the qualification standards and functional statements of assigned grade.
The focus of care will be centered on Geriatric Veterans utilizing an Interdisciplinary team approach.
