Committed to fostering a culture of compassion and well-being, Radiant Health seeks a dynamic and experienced professional to join its team as Case Coordinator for our Community Support Program.
As part of the treatment team, the employee in this position functions as Case Coordinator for adults diagnosed with Severe and Persistent Mental Illness and their families within the Community Support Program.
The employee acts as case coordinator to ensure that continuity of care is provided to each client.
Assess client's level of functioning, strengths, and needs.
Provide person-centered planning with regard to client level of need in a timely manner.
Coordinate treatment skills training with a team of Skills Specialists to implement.
Assess and make appropriate arrangements for interventions and referrals based on client need.
Participate in planning with appropriate family and support with, and on behalf of, client.
Employees shall function in compliance with all organizational policies and procedures as well as local, state, and federal regulations and observe ethical standards and confidentiality standards at all times.
MINIMUM QUALIFICATIONS BS/BA, BSW, (BSW preferred) in mental health discipline.
Have a minimum of one (1) year experience working with severe and persistent mental illnesses.
Superior computer abilities in using electronic medical Records (EMR) Negative Mantoux Have or be willing to obtain CPR Certification.
Must have sight and hearing to be able to observe client behavior and communicate with clients; must have own automobile to drive to and be ambulatory to visit the clients in their home which may involve climbing stairs.
Must be able to work full time.
